  The history of Cialis http://canamedic.com/in.cgi?2 cannot be discussed without mentioning Viagra (sildenafil), Pfizer's impotence drug. The FDA's approval of Viagra on 27 March 1998 was a ground-breaking commercial event for the treatment of impotence; sales exceeded a billion dollars.   In 1993, the Bothell, Washington, biotechnology company ICOS Corporation began studying compound IC351, a phosphodiesterase type 5 (PDE5) enzyme inhibitor. In 1994, Pfizer scientists discovered that sildenafil citrate, which also inhibits the PDE5 enzyme, caused penile erection in men participating in a clinical study of a heart medicine. Although ICOS scientists were not testing compound IC351 for treating impotence, they recognized its potential usefulness for treating said disorder. Soon, in 1994, ICOS received a patent for compound IC351 (structurally unlike sildenafil and vardenafil), and Phase 1 clinical trials began in 1995. In 1997, the Phase 2 clinical studies were initiated for impotent men, then progressed to the Phase 3 trials that supported the drug's FDA approval.
  In 1998, ICOS Corporation and Eli Lilly and Company formed the Lilly ICOS, LLC, joint venture company to further develop and commercialize tadalafil as a treatment for erectile dysfunction. Two years later, Lilly ICOS, LLC, filed a New Drug Application with the FDA for compound IC351 (under the tadalafil generic name, and the Cialis http://canamedic.com/in.cgi?2 brand name). In May of 2002, Lilly ICOS reported to the American Urological Association that clinical trial testing demonstrated that tadalafil works for up to 36 hours; one year later, the FDA approved tadalafil. One advantage Cialis http://canamedic.com/in.cgi?2 has over Viagra and Levitra is its 17.50-hour half-life [1] (thus Cialis http://canamedic.com/in.cgi?2 is advertised to work for up to 36 hours, after which time there remains ca. 25% of the absorbed dose in the body) when compared to the 4.0-hour half-life of sildenafil (Viagra).   In 2007, Eli Lilly and Company bought the ICOS Corporation for $2.3 billion dollars. As a result, Eli Lilly owned Cialis http://canamedic.com/in.cgi?2 and then closed the ICOS operations, ending the joint venture and firing most of ICOS's ca. 500 employees, except for 127 employees of the ICOS biologics facility, which subsequently was bought by CMC Biopharmaceuticals A/S(CMC).
  Persons surnamed "Cialis" objected to Eli Lilly and Company's so naming the drug, but the company insisted that the drug's trade name is unrelated to the surname. [4]

  Although Sildenafil (Viagra) and Vardenafi (Levitra), and Cialis http://canamedic.com/in.cgi?2 (Cialis) all work by inhibiting PDE5, Cialis's pharmacologic distinction is its longer half-life (17.50 hours) — compared to Viagra (4.0–5.0 hours) and Levitra (4.0–5.0 hours) — resulting in longer duration of action, and so partly responsible for "The Weekend Pill" sobriquet. Furthermore, the longer half-life is the basis for current investigation of Cialis's daily therapeutic use in relieving pulmonary arterial hypertension. Currently, sildenafil (trade name Revatio) is approved in several world regions as a thrice-daily therapy for pulmonary arterial hypertension.
  Penile erection during sexual stimulation is caused by increased penile blood flow resulting from the relaxation of penile arteries and the smooth muscle of the corpus cavernosum. This response is mediated by the release of nitric oxide (NO) from nerve terminals and endothelial cells, which stimulates the synthesis of cGMP in smooth muscle cells. Cyclic GMP relaxes smooth muscle and increases blood flow to the corpus cavernosum.

  TADALAFIL (Cialis®) is used to treat erectile dysfunction (ED) in men. Tadalafil does not cure ED or increase a man's sexual desire. Tadalafil is only for men with ED and is not for women or children. Tadalafil should only be used under a healthcare provider's care.
  
  What should I tell my health care provider before I take this medicine?
  
  They need to know if you have any of these conditions:
  
  anatomical deformity of the penis, Peyronie's disease, or ever had an erection that lasted more than 4 hours
  bleeding disorder
  diabetes
  frequent heartburn or gastroesophageal reflux disease (GERD)
  heart disease, angina, a history of heart attack, irregular heart beats, or other heart problems
  high or low blood pressure
  high cholesterol
  HIV infection
  kidney disease or require dialysis
  liver disease
  sickle cell disease
  stroke
  stomach or intestinal ulcers
  eye or vision problems or a rare genetic eye disease called retinitis pigmentosa

  How should this medicine be used?
  
  Take tadalafil tablets by mouth. Follow the directions on the prescription label. The dose is usually taken 30 minutes to 1 hour before sexual activity. You should not take this dose more than once per day. Swallow the tablets with a drink of water. You may take tadalafil with or without meals. Do not take double or extra doses. Do not change your dose without talking to your prescriber. Your prescriber may lower or raise your dose, depending on how your body reacts to tadalafil. If you have kidney or liver problems or are taking certain medications, your prescriber may limit your dose of tadalafil.
  
  Contact your pediatrician or health care professional regarding the use of this medicine in children. Special care may be needed. At this time, this medicine is not for use in children.
  
  What if I miss a dose?
  
  This does not apply.
  
  What drug(s) may interact with Tadalafil?
  
  Do not take tadalafil if you are taking the following medications:
  
  nitroglycerin-type drugs for the heart or chest pain such as amyl nitrite, isosorbide dinitrate, isosorbide mononitrate, nitroglycerin, even if these are only taken occasionally
  Tadalafil may also interact with the following medications:
  
  .alpha blockers, such as alfuzosin (UroXatral®), doxazosin (Cardura®), prazosin (Minipress®), or terazosin (Hytrin®), used to treat high blood pressure or an enlarged prostate.
  
  bosentan
  certain antibiotics such as clarithromycin, erythromycin, troleandomycin
  certain drugs used for seizures such as carbamazepine, phenytoin, and phenobarbital
  cimetidine
  cisapride
  diltiazem
  grapefruit juice
  medicines for fungal infections (fluconazole, itraconazole, ketoconazole, voriconazole)
  mibefradil
  nicardipine
  certain medicines for the treatment of HIV infection or AIDS
  quinidine
  rifabutin, rifampin or rifapentine
  some drugs for treating depression, anxiety or other mood problems (examples: fluoxetine, fluvoxamine, nefazodone)
